   1.    Fill out the online Rebate form at www.promedia-tech.com/pc8rebate0731.htm

    2.    Print out the form before clicking on Submit.  After print out click on Submit.

   3.    Attach a copy of your "original sales receipt or invoice that shows purchase price" dated 07/01/04 thru

           07/31/04 from PC Mall, Mac Mall, or ecost to the printed rebate form.

   4.    Attach the Original UPC barcode symbol cut from the package.  Do not send in the UPS

           label this will not be accepted!  No photocopies will be accepted.     

   5.    Sign and date the rebate form.
